Application Process

What every applicant should know:

  • You must report all annual household income received (earned and unearned).
  • You must list and report each household member and their Social Security number.
  • You may enroll online or by submitting a paper application. Paper applications submitted to Insure Oklahoma for processing can take up to 30 days to complete.
  • A completed subscriber application must contain true and accurate information.
  • All applications are subject to review and audit for accuracy.
  • Eligibility may be reversed at any time if inconsistencies are found.
  • All monies paid in error will be subject to recoupment.
  • If you are eligible your eligibility will start 45 – 60 days from date of application providing you have submitted all requested documents by deadlines.

***Late payments received and cashed do not reinstate the Insure Oklahoma policy absent reapplication and acceptance by Insure Oklahoma. Any funds received for policies which are not subsequently reinstated will be returned and do not constitute an agreement for coverage nor will they be applied to previous periods occurring between cancellation and possible reinstatement. ***

How to get application information:

  • If an individual chooses to apply, they may either apply on the Web site, or download the application form from the Web site and submit it to Insure Oklahoma for processing.

  • Submitting a paper application will result in an effective date 1 to 2 months later than if applying on line as there is a 30 day processing time for Insure Oklahoma processors.
  • Completing a paper application before applying online will ensure that you have all information needed in advance to going online and applying. The system will time out after 20 minutes of inactivity.
