What can the Insure Oklahoma Individual Plan do for me?

Individuals who participate in the Individual Plan gain access to an affordable health care option.

Who is the Insure Oklahoma Individual Plan available to?

The Insure Oklahoma Individual Plan is available to those persons who meet the definition in one of the following groups:

Group one is defined as: Working adults who are not eligible for an employer-sponsored Qualified Health Plan, and who work for an Oklahoma business with 99 or fewer employees;

Group two is defined as: Temporarily unemployed adults who are eligible to receive unemployment benefits through the Oklahoma Employment Security Commission (OESC);

Group three is defined as: Working adults with a disability who work for any size employer and have a ticket to work. Please call 1-888-365-3742 for details.


Individual Employee Eligibility Criteria

In order to participate in the Insure Oklahoma Individual Plan the individual must:

  • Be in one either group one, group two, or group three;
  • Be an Oklahoma Resident;
  • Be an U.S. Citizen (or qualified alien);
  • Be between the ages of 19 and 64;
  • Not be enrolled in or have an open application for Medicaid or Medicare;
  • Not be eligible for enrollment in the Insure Oklahoma employer sponsored insurance program;
  • Provide their Social Security Number for all household members;
  • Have an annual GROSS household income within the eligibility guidelines*

*Proof of income may be required
